AI Workflow Failure-Mode Audit
This fictional sample shows how Zinvyl evaluates one bounded AI-enabled workflow without production access, autonomous changes, or claims of whole-system safety.
Effective October 23, 2026
Synthetic workflow
A fictional support assistant classifies a sanitized request, retrieves approved knowledge, drafts an answer, checks a policy rule, and sends the draft to a human reviewer. No customer data, credentials, production system, or regulated decision is involved.
Example failure register
- Retrieval omission: a documented policy source is absent from one test context; the finding cites the test identifier and records uncertainty.
- Approval bypass risk: one simulated branch reaches the release boundary without the expected reviewer marker; immediate containment is to disable that branch.
- Unsupported confidence: the draft states a conclusion more strongly than the supplied evidence permits; the remediation adds an evidence-threshold rule.
Delivered package
- An audit manifest identifying the reviewed steps, supplied evidence, exclusions, and unresolved inputs.
- An evidence-cited failure register with severity, likelihood, affected step, containment, and uncertainty.
- A prioritized remediation plan reviewed by an accountable human before release.
Boundary
The service covers one workflow of no more than eight documented steps and up to ten sanitized evidence excerpts. It does not include production access or writes, code changes, deployment, live monitoring, model training, security certification, regulated or high-impact decision review, legal advice, or guaranteed improvement.
